National Environmental Leadership Award in Asthma Management

EPA is no longer accepting applications for the 2008 National Environmental Leadership Award in Asthma Management. Below is a description of the 2008 application instructions. Reviewing this information may help you prepare for the 2009 awards. Keep in mind, however, that EPA may modify program details based on this year’s experience and program direction.   • Application

    The key to winning the National Environmental Leadership Award in Asthma Management is to demonstrate that your comprehensive asthma management program is consistent with the NIH Guidelines throughout the continuum of care, includes a robust environmental component, and achieves positive health and financial outcomes. Your responses to the five areas in this application will be evaluated against criteria designed to discover and highlight national leaders. You may collaborate with your partnering organizations on an application and you are encouraged to highlight key roles that these organizations play in delivering quality care.

    Evaluation Criteria

    Applications will be reviewed by a panel composed of federal and non-federal representatives with expertise in asthma care, health care policy, quality of care, and economics. Applicants will be judged solely on the information in their applications. In making award decisions, the panel will use the criteria described here to evaluate applications. These criteria reflect the importance of an integrated, comprehensive asthma management program — as well as the focus of this award on Environmental Leadership in Asthma Management.

    Sample Responses

    To assist applicants in determining the type of information to include and the appropriate level of detail, we have provided sample responses for a hypothetical health plan and health care provider. Although elements described in the sample responses may represent actual program elements, the programs as a whole do not exist. The sample responses are provided solely for your information; they are not intended to endorse particular activities. As an additional reference, you may also review the program summaries of past award winners.

Review Application Requirements
A complete application consist of a cover page, complete responses to the five application areas, and optional supplemental materials.

The paragraph requested on the cover page may not exceed 150 words; the responses to the five areas may not exceed 5,000 words (or ten pages); and the supplemental materials may not exceed ten pages.

Page limits are not cumulative. You may not, for example, submit five pages of supplemental material and 15 pages responding to the criteria. A counter is provided in the online application to help you keep track of the length of your submittal.

Supplemental Materials
You may attach supplemental materials that document or provide additional support of the information in the application if they are:

  • Clearly referenced and their relevance is explained in the body of the application. Attachments that are not referenced, explained, and given a title will not be considered during the evaluation;
  • Electronic files (e.g., Word, PDF, JPG, PowerPoint) that can be printed and copied for distribution to the evaluation panel; and
  • Not in excess of 20 megabytes per file and ten pages total.

Hard copies of brochures or other materials will not be circulated to the evaluation panel. Applicants submitting an application by mail must limit their attachments to documents of ten pages or less. The attachments must be capable of being copied without having to remove bindings or other fasteners. In addition, you must prepare a table that lists the five areas and the title (not the file name) of any supplemental materials associated with each area. Applicants submitting online will be prompted for this information.
